Quarter 2

ASB would like to fund projects and causes that promote school spirit and support learners in the classroom and beyond! The selection committee is made up of our ASB Executive Board, Activities Directors, and Administration. We will fund as many projects as possible. Please limit your answers below to no more than two paragraphs for each question. The panel will contact your president, advisor, or staff member for more information if necessary throughout the review process.

This form must be submitted by Club Advisors or PHS Staff Members by mid-December. The form will be released very soon!

View Your Club Status (only accessible through IUSD Google Account)

Your club status will be updated at the end of each month. If you reach 2 strikes, your club advisor will be notified by email. If you reach 3 strikes, your club will be terminated, and club advisors will be notified by email regarding your club's termination. Please refer to the Club Advisor Handbook for more information.

Disbanded/Terminated/Inactive Clubs:

Any terminated, disbanded, or inactive club must complete the club application process again during the following school year to be reinstated. No terminated, disbanded or inactive club is permitted to meet for the duration of the year in which they are terminated, disbanded or inactive. No students may claim ownership over any terminated, disbanded or inactive club at any point in time. If a club feels it has been terminated without reason, it may appeal to the administration and the school leadership class for reinstatement.

Club Advisors are responsible for filling out any form regarding Club Financials. Club Advisors must submit all forms. All club financials will be reviewed on Wednesday during fourth period. For more information, please refer to the Club Financials page.
