COVID-19 Requirements

Winter Formal will be classified as a “Mega Event” as defined in the new CDPH guidance to be posted on January 15. A Mega Event is a gathering or activity that draws more than 500 attendees indoors or 5,000 attendees outdoors, according to CDPH guidance posted on Dec. 31. For indoor Mega Events, attendees must show verification of fully vaccinated status or a negative COVID-19 test result before they enter, and all guests must adhere to the latest CDPH guidance for face coverings. Any student (including guests) attending the Winter Formal must provide proof of vaccinated status or a negative COVID-19 test result before they enter.

How can I meet the CPPH COVID-19 safety guidelines?

  • I am fully vaccinated for COVID-19. A COVID-19 booster shot is NO LONGER REQUIRED for this event.

  • I have had COVID-19 within 90 days of February 5th (11/5/2022 or later) and it can be verified by school records.

  • I will provide a negative test verification that adheres to CDPH guidelines. Taking an at-home rapid test is not allowed because of lack of documentation.

    • Students who selected this option can get a test from Portola High School. Pick up the test outside the health office on Tuesday, February 1st and Wednesday, February 2nd. Take the test Thursday morning, drop off the test before 10 AM on Thursday, February 3rd.

    • Take a PCR test at a testing site within 48 hours of the dance. Bring documentation of a negative result to the dance.

    • Take a rapid antigen test at a testing site within 24 hours of the dance. Bring documentation of a negative result to the dance.

Please fill out the Winter Formal COVID-19 Safety Form to verify that you meet the CDPH COVID-19 safety guidelines and show proof of vaccination ahead of time. The Google Form must be completed by Friday, February 4, 2022 @ 3 PM.

**Guests must fill out the Winter Formal COVID-19 Safety Form. If the guest cannot access the Google Form, the individual inviting the guest must fill it out for them. You may put "GUEST" under "Short ID Number"**

Tickets are on sale in the ASB Office and PHS Webstore beginning January 17, 2021.

If you bought the Dance Package, you do not have to buy a dance ticket.

Guests will be allowed to come to Winter Formal. To bring a guest to the dance, a PHS student needs to submit a signed Guest Contract (under Resources) and Photocopy Of The Guest's Picture ID to the Activities Office by Wednesday, February 2, 2022 @ 3:30 PM. PHS students can pick up a guest contract from the Activities Office. The price for a guest ticket is $85.

All guest tickets have to be bought ahead of time. Tickets will not be sold at the door.
